Nicotine-based vapes (e-cigarettes) are likely to cause cancers of the lung and oral cavity, according to a new study led by researchers at UNSW Sydney and published in the journal Carcinogenesis.

Described by lead author Bernard Stewart as the most definitive assessment of e-cigarettes to date, this study draws on a wide range of global evidence, including clinical data, animal studies, and laboratory research. While vaping has often been framed primarily as a gateway to smoking, the study shifts focus to its potential to cause cancer independently, finding consistent signals across multiple scientific disciplines.

The researchers identified numerous carcinogenic substances in e-cigarette aerosols, including volatile organic compounds and metals released from heating elements. Evidence from human biomarkers showed DNA damage, oxidative stress, and inflammation, while animal experiments demonstrated the development of lung tumors. Laboratory studies further revealed disruption of cellular processes linked to cancer. The convergence of these findings led the authors to conclude that the link between vaping and cancer is increasingly compelling.

The study also highlights broader public health concerns, particularly the rapid uptake of vaping among young people and the persistence of “dual use,” where individuals both vape and smoke. According to co-author Freddy Sitas, this pattern may significantly amplify cancer risk, with some evidence suggesting a four-fold increase in lung cancer among dual users. Drawing parallels with the slow recognition of smoking-related harms in the past, the researchers warn against delaying action on vaping, arguing that waiting decades for definitive long-term data could repeat historical mistakes.
