A new global analysis has revealed that nearly a third of people who develop one of 17 major cancers may die before receiving an official cancer diagnosis. The danger is not only that cancer is rising worldwide; millions of cases may never enter the medical record at all.

Published in The Lancet Oncology journal, this study highlights how many cancer patients remain effectively invisible to healthcare systems, particularly in poorer regions where access to specialists, scans, laboratories, and referral networks is limited.

To carry out the study, the researchers used a computer-based “microsimulation” model to estimate both total cancer cases and how many are actually diagnosed. They found that failures in diagnosis often begin long before treatment, as patients may struggle to access primary care, face long waits for scans or tissue testing, or be blocked by cost, travel difficulties, fear, and overstretched health services. By 2050, the researchers estimate that nearly half of diagnosed cancers will still only be detected after the disease has already spread extensively, making treatment far more difficult.

The study also revealed enormous global inequalities. In Western Europe, less than 1 percent of people with new cancers are projected to die undiagnosed, while in western Africa the figure could reach more than 67 percent. Researchers say the findings expose a hidden cancer burden that is far larger than official statistics suggest, especially in low- and middle-income countries.
