Drugs such as semaglutide and tirzepatide have become enormously popular for weight loss, but emerging evidence suggests that their effects on appetite and digestion may have potentially serious nutritional consequences in some patients. Researchers have identified cases of Wernicke encephalopathy, a rare but potentially fatal brain disorder caused by severe deficiency of vitamin B1 (thiamine), in people using GLP-1-based drugs.

Thiamine is essential for energy production and normal functioning of the brain and nervous system. Severe deficiency can cause confusion, problems with coordination and abnormal eye movements and, if not treated quickly, may result in permanent brain damage or death. The reported cases have generally involved prolonged vomiting, loss of appetite, greatly reduced food intake, or rapid weight loss. While the evidence does not prove that GLP-1 drugs commonly cause Wernicke encephalopathy, it does highlight the importance of monitoring nutritional status when pharmaceutical treatments substantially alter eating and digestion.

Drug-induced nutrient depletion is far from a new phenomenon. Scientific evidence has long shown that various commonly used medicines can reduce the body’s supply of essential vitamins, minerals and other micronutrients. Yet this problem receives surprisingly little public attention. The latest GLP-1 findings provide another reminder that when drugs alter normal biological processes, their effects on nutritional status should never be overlooked.
